Overuse Injuries in Youth Athletes: Why They're Rising and How to Prevent Them
Youth sport has changed. Where previous generations played multiple sports casually across the seasons, today's young athletes are increasingly specializing early, training year-round, and competing at intensities that were once reserved for adults. The result has been a significant rise in overuse injuries, and they're now one of the most common reasons young athletes end up on the sideline.
What Is an Overuse Injury?
Unlike acute injuries that happen from a single event like a fall, a collision, or a twist, an overuse injuries develop gradually from repetitive stress applied to tissue that hasn't had adequate time to recover. Tendons, growth plates, bones, and muscles are all vulnerable. Common examples include Osgood-Schlatter disease at the knee, Little Leaguer's elbow, stress fractures, patellar tendinopathy, and Sever's disease at the heel. Many of these are unique to young athletes precisely because growing bones and soft tissue respond differently to load than mature adult tissue.
Why They're Rising
Early sport specialization is one of the biggest drivers. Research consistently shows that athletes who specialize in a single sport before age 14 are at significantly higher injury risk than those who play multiple sports. Multi-sport participation builds varied movement patterns, distributes physical stress across different tissues, and reduces the repetitive loading on the same structures season after season.
Training volume is the other major factor. Many youth programs have adopted adult training models without accounting for the demands of growth and development. When load increases faster than the body's ability to adapt, particularly during growth spurts when bone grows faster than surrounding soft tissue, injury risk rises sharply.
How to Prevent Them
The single most evidence-supported recommendation is to encourage multi-sport participation through early adolescence and delay specialization where possible. Beyond that, managing training load is critical, total weekly hours, session frequency, and intensity should increase gradually. Additionally, athletes should have at least one to two full rest days per week and a meaningful off-season each year.
Strength and movement quality matter too. Young athletes who develop foundational movement skills like landing mechanics, single-leg stability, and hip and core strength are better equipped to handle sport-specific demands without breaking down.
Parents and coaches play a significant role. Normalizing rest, recognizing early warning signs of overload like persistent joint pain, fatigue, declining performance, loss of enthusiasm, and resisting the pressure to push through discomfort are all part of keeping young athletes healthy and in sport for the long term.

Any pain in a young athlete that persists beyond a few days, worsens with activity, or involves a joint should be evaluated by a healthcare provider. Growth plate injuries in particular can have long-term consequences if missed or managed poorly. Early assessment leads to better outcomes and shorter time away from sport.
